*Deliveries set up by 2pm are eligible for same day delivery, after 2pm will be next business day delivery

*One FREE delivery per address/patient per week, additional deliveries are $5 per delivery

*Payment for prescriptions must be set before delivery

• -may pay over the phone
• -may give exact change to delivery driver
• ^driver does not carry cash/change
• -may give a check to delivery driver

*Someone aged 18 or older must be home at time of delivery

*Deliveries are Monday thru Friday, 3pm-6pm

*It is your responsibility to make sure we have the correct delivery address and phone number; incorrect information may lead to prescriptions being returned to the pharmacy

*Failed delivery attempts will be returned to the pharmacy, and it will become the patient’s responsibility to arrange pick up of that prescription
